Free pre-screening call
A registered nurse on our clinical team has an unhurried conversation with you about your current arrangements. Ten minutes, free, no commitment.

Records request
With your consent we request a clinical summary from your previous practice through the appropriate channels. Your new doctor reads it before your first consultation.

First consultation
An unhurried appointment with an Ahpra-registered medical practitioner to confirm your history, discuss what was working, and agree the next steps that suit you.
What to bring to the pre-screen call
- The name and contact details of your current clinic — that's all we need to start the records request.
- Any current treatment plan documentation you have on hand, including the date of your most recent consultation.
- A short list, in your own words, of what's been working and what hasn't. Helps the nurse capture the picture quickly.
- Your Medicare card number — used only for verification under the Australian Privacy Principles.
You don't need to have a reason for moving, or any of your previous records in hand. We do the chasing.
